Preventive Health Care Plans for Dogs

We have 4 different Preventive Health Care Plans for Dogs:   Puppies, Basic Adult, Adult with Oral Diagnostic and Treatment Procedure, and Senior/Special Needs Dogs.  "Special Needs" dogs are ones that need more frequent examinations or more detailed monitoring due to on-going illnesses occuring before they are over 10 years of age, which is the general recommendation for Senior Testing.  Below is a listing of the services included in each plan; there is a $50 enrollment fee although this is waived in successive years as long as you renew your membership without lapse.  You can avoid this fee entirely by pre-paying for the entire plan in advance. To enroll, print and complete the agreement (click on Preventive Care Plan Agreement at left), then bring to your next appointment or contact our office at 574-654-3129. For more description, be sure to read the information on the main Preventive Health Care Plan tab at left. 

Preventive Health Care Plan for Puppies

Annual Services include:

Exams*:

      3 Comprehensive Physical Examinations

      2 Technician Consultation/Exams

      1 Progress Examination

Vaccinations:

      1 Rabies Vaccine

      4 Puppy Vaccination series and series final (Distemper, Parainfluenza, Hepatitis, Parvovirus)

      1 Kennel Cough (bordatella) Vaccine

Lab Work:

      3 Intestinal Parasite Exams (Microscopic Fecal Analysis)

Parasite Control:

      3 Intestinal Deworming Treatments

      12 months Heartworm & Flea & Tick Prevention (also prevents most intestinal worms) with Heartgard and Nexgard or Bravecto.   Some substitutions permitted.  Additional costs for dogs over 100 lbs will be the responsibility of the owner

Surgical Procedure:

       Preferred Plan Spay or Neuter including IV fluids, multiple types of pain management, therapy laser treatment, and advanced monitoring and supportive care.  Since we are recommending later spay/neuter for large breed dogs, these procedures can be postponed but still covered up to 18 months of age as long as no pregnancy occurs

       10% discount for surgical laser use and pre-operative CBC, blood profile, and clotting tests

Additional Services:

       Microchip Permanent Identification with lifetime registration

       4 Nail Trims***

10% discount on all products and services not included in the plan

Total Retail Price if purchased separately:  $1244.50

Monthly Payment only $78 on the plan so minimum savings of $308.50!

  2 Intestinal Parasite Exams (Microscopic Fecal Analysis

  1 Early Detection CBC (Red, White and Platelet cell counts), Miniprofile (blood sugar, liver, kidney, protein tests), Thyroid test, Heartworm test (must be done within 6 months of the Oral Diagnostic and Therapeutic Procedure and lab submission requirements necessitate that it be done prior to the procedure)

Parasite Control:

  2 Intestinal Deworming Treatments

  12 months Heartworm & Flea & Tick Prevention (also prevents most intestinal worms) with Heartgard and Bravecto. Some substitutions permitted.  Additional medication required for dogs weighing over 100 lbs will be at the expense of the owner

Surgical Procedures:

   Oral Diagnostic and Treatment Procedure including anesthesia with IV fluids, monitoring, hospitalization, ultrasonic and manual dental scaling, polishing, fluoride treatment, and Oravet Barrier Sealant

   Full Mouth X-rays (with no further cost if additional views are taken at the same visit)

   10% discount on dental therapy such as surgery, extractions, pain medications, antibiotics, and home care products including TD

Additional Services:

4 Nail Trims***

10% discount on all products and services not included in the plan

Monthly Payment on the plan is only $74 for a minimum savings of $244!

 Preventive Health Care Plan for Dogs: Seniors and Special Needs Pets

Annual Services include:

   1 Semi-annual Senior Examination 

Vaccinations** (core vaccines as are due):

  1 Rabies Vaccine

  1Distemper, Parainfluenza, Hepatitis, Parvovirus Vaccination

  1 Kennel Cough (bordatella) Vaccine

Lab Work:

  2 Intestinal Parasite Exams (Microscopic Fecal Analysis)

  1 Heartworm Test

  1 Early Detection Profile with CBC (Red, White and Platelet cell counts), General profile (blood sugar, liver, kidney, protein, pancreatic, electrolyte and other tests),  Thyroid tests

  1 Complete Urinalysis with sedimentation exam

  1 Urine Protein:Creatinine Ratio

  1 Urine Culture Screen 

  1 Laboratory Collection Fee

Parasite Control:

  2 Intestinal Deworming Treatments

  12 months Heartworm & Flea Prevention (also prevents most intestinal worms) with Heartgard and Bravecto. Some substitutions permitted. Additional medication requirements for dogs weighing over 100 lbs will be the responsibility of the owner

  1 Intraocular Pressure Measurement (glaucoma screening)

Monthly Payment on the plan of only $72 for a minimum savings of $254.30!

*Appointments for examinations must be made at least 48 hours in advance, in regular time slots (same day emergency appointments are not covered).   Missed appointments without 48 hours advance notice of cancellation, including for drop-off services, will be billed as though they were provided.

**No discounts are given during those years when multi-year vaccines are not due.

Sorry, but due to the extent of the discount, additional discounts for multiple pet or senior citizens are not available.  No refunds for unused products or services.  Discounts are only available for products used for the pet enrolled in the plan.

*** Level III nail trims, or sedation required to perform nail trims, are not included
